This Notice of Privacy Practices describes how Behavioral Wellness Clinic (“BWC,” “we,” “our,” or “us”) may use and disclose medical and personal information about you and how you can access that information. Protected Health Information (PHI)
Your health record contains personal information about you and your mental or physical health. This information, which may identify you and relates to your past, present, or future health condition or related care, is known as Protected Health Information (PHI).
BWC and its clinicians are required by law to:

PHI of minors is disclosed only with proper parental or guardian authorization, except where minors may legally consent to their own treatment.
Website, Forms & Digital Communications
In addition to PHI, BWC collects non-clinical personal information through its website and digital communications.
Information We May Collect

- Information submitted through contact forms, consultation requests, or newsletter sign-ups
- Appointment inquiries
- Website usage data such as IP address, browser type, and analytics
Website forms are not intended for urgent or emergency communications.
Consent Through Form Submission
